Access Health Management Report Form and enter their body temperature and information on their physical condition in the following report form.

健康管理報告の【入構　可】メールを管理室に転送する。(このアドレスは画像なのでコピーできません。)

Forward an email to the Clean Room Management Office requesting permission to enter the University of Tokyo.*The email address is an image and cannot be copied.

入室届フォームに入力する。

Access and complete Takeda CR entry form.

入口の手指消毒器で消毒、もしくはトイレで石鹸を使用して手を洗う。（入り口に印刷した入室届を置いてます。）

In Takeda bldg. 1F, wash your hands with alcohol or with water and soap in the toilets.

一階ホールで待機し、CR入口、前室、控室、更衣室が二人以下か確認する。(サーモグラフィにより体温を監視しています)

Still at the 1st floor, check on the monitor the situation at the CR entrance, the front room, the waiting room, the dressing room.Get your body temperature monitored by thermography.

モニター前のサニメント手袋を両手にはめる。

Put on saniment gloves (in front of the monitor).

２階へ移動する。(2人以下の少人数で移動すること)　
接触を減らすために、階段での移動が望ましい。

Go down to basement 2 (2 persons max *).
